We provide case management services for the elderly, disabled, and mentally ill. Delivered under the Medicaid home and community based waiver program, our case management services meet the needs of consumers who, in the absence of these services, might need to be in a nursing home. We provide our services wherever our clients need them, including:

• In their own homes
• Foster homes
• Personal care homes
• Assisted living facilities

The Montana Department of Public Health and Human Services, Senior and Long Term Care Division limits the number of individuals our case management team is allowed to serve.
