Home care services provided by RiverStone Health assist patients recovering from illness or surgery. Comprehensive services are provided, under the direction of a physician, to patients in their own home and may include:
- Skilled nursing
- Physical therapy
- Occupational therapy
- Speech therapy
- Medical social work
- Home health aide
RiverStone Health Home Care Services is the only home care agency in Montana nationally accredited by the Community Health Accreditation Program (CHAP). Created in 1965, CHAP was the first to recognize the need and value for accreditation in community-based care and is the oldest national, community-based accrediting body with more than 5,000 agencies currently accredited nationwide. CHAP accreditation assures the highest standards and quality of care is provided to our homecare patients.
Home care services may be requested by physicians, the patient or family member, or a hospital discharge planner by calling during business hours. Through consultation with the patient’s healthcare provider a plan of care is developed to meet the patient’s individual healthcare needs.
Cost of services may be covered by Medicare, Medicaid, private insurance or private pay. Limited supplemental funding is available for those who qualify.
