Diabetes Education Program
Patient education is a key component of managing and controlling diabetes. RiverStone Health offers a recognized diabetes education program through the American Diabetes Association. Registered dietitians and registered nurses provide individual counseling on how to successfully self-manage diabetes. Education sessions include information on the diabetes disease process and treatment options, nutritional management, physical activity, medications, monitoring blood glucose, preventing and treating complications, and developing strategies to promote behavior change. 

Through the support of the healthcare team and increased knowledge and awareness of diabetes, a patient can assume a major part of the responsibility for his or her diabetes management. Additionally, self-management education prevents unnecessary hospital admissions and acute and chronic complications.
Patients can ask their primary healthcare provider for a referral to set up an appointment with a diabetes instructor or call 671.2687.

Diabetes Coalition
RiverStone hosts a coalition devoted to addressing chronic disease, specifically diabetes, in the community.  The “Choose Health” group has conducted patient and provider focus groups to identify ways to improve the quality and access to healthcare as well as improve diabetes outcomes.
